Requirements
- Must have minimum LSW.
- Licensed or registered in the state of practice as required by state regulations. ACSW/ASW licensure required in the State of California.
Responsibilities
- Conducts, reviews, and assesses quality of life survey results, develops patient goal setting, completes appropriate follow up to analyze patients’ plan to improve scores and quality of life.
- Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing.
- Counsels new and current patients and families on treatment options and modalities.
- Conceptualizes problems, interventions and the evaluation of interventions with respect to human behavior, personality development, and social environment.
- May be required to cover more than one facility in the area.
