Responsibilities
- Assesses the bio-psychosocial needs and abilities of residents and families
- Provides a range of Social Work services, interventions and education
- Utilizes personal, family and community resources to achieve and maintain an optimal level of physical, mental, social well-being and quality of life for the residents and families
- Completes bio-psychosocial assessments, care planning, counseling, advocacy for resident and family members
- Ensures safe and appropriate discharge planning
