Requirements
- Experience in social services or healthcare preferred, but we are willing to train the right candidate
- Compassionate, patient, and resident-focused attitude
- Excellent communication and organizational skills
- Ability to work well in a team environment
- Knowledge of community resources and basic computer skills a plus
Responsibilities
- Assist in the development and implementation of care plans that address residents’ social, emotional, and psychological needs
- Support residents and families with discharge planning, community resources, and emotional support
- Advocate for residents’ rights and work to resolve any concerns or issues
- Maintain accurate documentation and reports in compliance with state and federal regulations
- Collaborate with the interdisciplinary team to provide a holistic approach to resident care
- Help organize resident-centered activities and programs that promote social engagement and well-being
