Requirements
- Bachelor’s degree in Social Work or related field (required)
- Knowledge of discharge planning, Medicare/Medicaid regulations, and community resources
- Strong organizational, communication, and documentation skills
- Passion for working with elderly and post-acute populations
Responsibilities
- Develop and implement individualized care and discharge plans in collaboration with the interdisciplinary team
- Serve as a liaison between residents, families, and community agencies
- Conduct psychosocial assessments and maintain thorough documentation
- Participate in care conferences and ensure residents’ rights and needs are respected
- Provide support during admissions, discharges, and transitions of care
- Promote resident well-being through advocacy and person-centered support
Preferred Qualifications
- Minimum 1 year of experience in a skilled nursing or long-term care setting (preferred)
