Requirements
- High school diploma or equivalent.
- 3 years of relevant experience in a healthcare setting.
- Knowledge of medical terminology and patient care processes.
- Excellent communication and organization skills.
- Must have high energy level, flexibility, and a desire to learn.
Responsibilities
- Manages patient inquiries via phone, email, electronic health record portal, and face-to-face interactions.
- Provides detailed information on health conditions and treatment plans.
- Coordinates communication between patients and healthcare providers.
- Schedules patient appointments with doctors and specialists.
- Manages the calendar to avoid conflicts and ensure timely visits.
- Sends reminders and follow-up messages to patients.
- Educates patients on their health conditions, treatment options, and preventive care.
- Provides resources and materials approved by healthcare providers.
- Conducts follow-up to ensure understanding and compliance.
- Maintains patient records, including contact information and medical history.
- Processes prior authorizations and patient assistance program applications.
- Coordinates and documents all referral processing steps throughout multiple stages.
- Participates in quality improvement initiatives and utilization review activities.
- Implements corrective actions based on feedback and data analysis.
Preferred Qualifications
- Associate’s degree in healthcare administration or related field.
- 5 years of experience in patient care coordination.
- Experience with electronic health records (EHR) systems.
- Bilingual (English/Spanish)
- Certified Ophthalmic Assistant or Certified Ophthalmic Technician.
