Requirements
- Degree from an Accredited School of Nursing or a Bachelor’s degree in Nursing
- 4 – 6 years of related experience
- RN – Registered Nurse – State Licensure and/or Compact State Licensure required
Responsibilities
- Assesses, plans, and implements complex care management activities
- Develops and continuously assesses ongoing care plans/service plans
- Coordinates and manages care between the member and care provider team
- Monitors care plans/service plans and member status
- Identifies problems/barriers for care management
- Reviews member data to identify trends and improve quality care
- Reviews referrals information and intake assessments
- Performs telephonic, digital, home and/or other site outreach
- Collaborates with healthcare providers to facilitate member services
- Collects, documents, and maintains all member information
- Provides and/or facilitates education to members and their families/caregivers
- Partners with leadership team to improve care and quality delivery
- May precept clinical new hires and provide guidance
- Engages and assists New Hire/Preceptee during onboarding
- Engages in a collaborative process with People Leaders and cross functional teams
- Performs other duties as assigned
- Complies with all policies and standards
