Requirements

  • Scheduling experience in a healthcare setting, home health is a plus!
  • High School Diploma Required
  • Excellent Customer Service Skills
  • Previous experience with community service, client service or staffing experience
  • Knowledge or previous experience working in an office setting with computers, phones, facsimile, and other related tasks
  • Driving Required. Must have valid driver’s license and current auto insurance
  • Individual must have the ability to stand and work or have mobility through mechanical assistance

Responsibilities

  • Coordinate quality care and customer service to clients
  • Coordinate the assignment of appropriate staff to clients
  • Schedule client services and identify problem situations
  • Ensure HomeCare services are in compliance with agency philosophy, policies/procedures, and in accordance with Federal, State, and local licensing laws and regulations
  • Ensure proper documentation and record-keeping for agency payors
  • Maintain data integrity in client scheduling software system
  • Understand and ensure adherence to the agreed Plan of Care for each client served
  • Partner and communicate with all members of the client’s circle of care
  • Monitor and report on changes in the client’s Plan of Care, services, or condition as required
  • Conduct periodic home visits and safety checks
  • Review and determine client service schedules to ensure adequate staffing and processing completed visits for payment
  • Support recruitment, screening and selection of staff based on Agency and external referrals
  • Support all initiatives related to Branch Growth
  • Provide administrative support for key branch functions
  • Assume on-call responsibility as needed
  • Other duties as assigned

Preferred Qualifications

  • Prior experience delivering services to the elderly, disabled, or developmentally disabled preferred (per regulatory requirements)
