Requirements
- 3-5 or more years of acute care hospital experience
- 3-5 years of case management experience
- 1-2 years data entry/computer experience
- Active Registered Nurse license
Responsibilities
- Overseeing the operations and administration of case management service
- Supporting and monitoring the case management staff’s performance while providing coaching, training, and development opportunities
- Managing and resolving staffing needs as well as complex cases that are escalated by Case Managers
- Facilitating appropriate lengths of stay and reimbursement for all hospital admissions
- Directing and supporting Case Managers, Social Workers, and Case Management Extenders daily work activities
- Using variance data to identify opportunities for improvement
- Monitoring and auditing quality and performance to promote positive patient outcomes
- Developing and implementing departmental goals, plans, and standards
- Collaborating to provide interdisciplinary leadership including guidance and direction for clinical case management and discharge planning management
- Performing other related duties as assigned
