Requirements
- Bachelor of Science degree in Nursing.
- License to practice as a Registered Nurse in the State of Michigan.
Responsibilities
- Coordinates the implementation of the treatment plan identified by the health care provider in an ambulatory/outpatient setting.
- Performs assessment of clients to facilitate appropriate access and level of care required.
- Supports the designed process of care by triage, patient education, implementing consultations and referrals, participation in team conferences, discharge planning and telephone follow-up to clients.
Preferred Qualifications
- Six to twelve months of experience in ambulatory care or community health preferred.
