Requirements

  • Pharmacist license issued by the state in which the team member practices
  • Two certifications: Immunization and non-vaccine injectable
  • Bachelor’s Degree in Pharmacy
  • Strong customer service and organizational skills
  • Excellent communication (written and verbal) and interpersonal skills
  • Demonstrated ability to work effectively with providers, staff and patients
  • Detail-oriented with the ability to handle multiple demands simultaneously
  • Intermediate computer skills including the ability to navigate through a database and run reports

Responsibilities

  • Reviews, evaluates, and consults with patients and other health care providers on issues relating to drug related problems and optimal drug therapy
  • Checks to prevent allergic or adverse drug reactions and reports them through appropriate channels
  • Reviews and interprets medication histories in order to make recommendations regarding drug therapy
  • Reviews and evaluates drug related information to make recommendations regarding the most effective, cost-effective drug therapy
  • Clarifies the patient’s understanding of their therapy to ensure the best possible outcome
  • Assures medication needs of patients are met throughout the continuum of care
  • Prepares and distributes drugs in accordance with Wisconsin Administrative Code relating to the practice of Pharmacy
  • Provides work direction and guidance for technicians in performing technical pharmacy functions
  • Is responsible for checking technician’s work to maintain high standards of pharmaceutical care
  • Provides input into establishing and implementing written policies and procedures for all pharmacy operations
  • Assumes responsibility for operation of the department in the absence of the direct supervisor
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served

Preferred Qualifications

  • Life support training courses may be required dependent on department discretion
