Requirements
- Current RN or PT license in the state of employment
- Minimum of Three (3) years of experience as an RN or PT in an acute care setting; and at least 1 year of Home Care experience.
- Valid driver’s license and auto insurance in your name as a driver
- Capable of all physical demands
Responsibilities
- Ensure Start of Care compliance by our clinical team and focus on Outcome Monitors as assigned.
- Participate in all liaison/administrative on-call rotation, and patient staffing as needed to manage after hour’s team, staffing needs and patient related issues.
- Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution & follow up.
- Complete patient visits as needed and assist with any patient issues.
- Participate in managing cost of good working with monitoring supply costs, overtime, staffing, bonuses and visits per episode.
- Select and onboard staff.
- Collaborate with intake, providing daily direction, managing referral volume based on staffing availability.
- Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
- Provide oversight for liaison and clinical staff and ensure timeliness of completion of work/charting as well as identify education needs for the team.
- Manage On call, PTO, call offs & daily/weekly productivity review to ensure proper staffing coverage to meet the needs of the business in the field and internally.
- Ensure quality and safe delivery of home health services by our clinical team.
- Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
