Requirements
- MD or DO degree from fully-accredited medical or osteopathic school of medicine
- Residency trained in internal or family medicine
- Excellent communication and inter-personal skills
Responsibilities
- Ambulatory primary care for older adults (60 and up)
- Innovative patient-centered care
- Collaborative open workspace
- No hospital or facility responsibility
- Office hours, Monday through Friday, 8:00 am to 4:30 pm
- 7 to 10 patients per day
- Dedicated MA for each provider
- One RN who manages transitions of care (hospital, ED, facilities)
- Night and weekend call-shared with other physicians (minimal calls)
