Physician Assistant – Advanced Clinical Provider – Hospital at Home Program – Open

Requirements

• Graduate of an accredited PA program
• Current NYS Physician Assistant’s License
• NCCPA certification
• DEA Certification
• BLS/ACLS/PALS certification
• At least one year of relevant clinical experience as a physician assistant

Responsibilities

• Provide clinical care to HAH patients under the direction of the Hospital at Home Medical Directors, Supervising Physician, and Associate Chief Advanced Clinical Provider
• Deliver high-quality clinical care to HAH patients across emergency departments, inpatient units, and the Virtual Unit
• Manage the full continuum of care for HAH patients, including enrollment, admission, daily clinical evaluations, discharge, consultative services, clinical monitoring, and telehealth visits
• Demonstrate an ability to make appropriate, highly complex clinical decisions and take effective action
• Perform on-going assessment of therapy and modify patient management plan as necessary
• Collaborate with interdisciplinary team members to ensure continuity of care and optimize the use of hospital resources
• Assists the physician as directed in the training of health personnel in diagnostic, therapeutic and clinical techniques
• Participate in continuing education and professional development activities to maintain clinical excellence

Preferred Qualifications

• A minimum of 8-10 years of Clinical experience in Inpatient Hospitalist Medicine and/or Emergency Medicine
