Requirements
- Graduate of an ACPE accredited Doctor of Pharmacy degree program or graduate
- Bachelor of Science in Pharmacy
- Master’s Degree or significant clinical practice experience (e.g., minimum of 3 years of clinical/direct patient care)
- The applicant also must have a Louisiana Pharmacy Intern license prior to start of the program
- Residents must obtain licensure to practice pharmacy in Louisiana before October 1 of the year starting the residency
Responsibilities
- Prepares, dispenses and manages medications to support safe and effective drug therapy for patients
- Instructs patients in the proper use of prescribed drugs and makes patient care rounds with physicians to evaluate patient progress
- Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes
- Collaborates with the health care team to provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens and multiple medications following a consistent patient care process
- Participates in collection of data for drug utilization evaluations (DUEs) and medication education to inpatients/out-patients
- Residents are expected to complete a research project by demonstrating the ability to evaluate and investigate practice, review data, assimilate scientific evidence to improve patient care and/or the medication use system
- Provides general and patient specific drug information to patients, physicians, nurses and other health care professionals
- Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients
Preferred Qualifications
