Requirements
- Maintain strict adherence to all Byram Healthcare policies and procedures as published and as amended from time to time
- Reviews, receives/verifies all new patient orders and documents
- Assists physician’s and other healthcare professionals involved in planning and implementing patient care
- Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding medications and their use
- Instructs/counsels patients and caregivers regarding specific drug therapy including possible adverse drug reactions and/or interactions
- Identifies appropriate outcomes of drug therapy
- Monitors and documents ongoing drug therapy including the assessment of therapeutic appropriateness of the choice of drug(s), therapeutic duplication within patient’s drug regimen, appropriateness of dose, frequency and route of administration, adherence to drug regimen, potential drug/food/or diagnostic test interactions, disease limitations to proper drug use
- Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals
- Responsible for ordering, inventory, maintenance of medications
- Prepares, labels, packages, and dispenses medication orders
