Requirements
- Bachelor’s degree in healthcare administration, business administration, public health, dental hygiene, or a related field
- Three years of experience in patient care coordination in a healthcare environment or designing or administering a public health program or dental education program
- Strong analytical, problem solving, communication, and organizational skills
- Ability to manage multiple tasks and work collaboratively with various departments
- Experience with word processing, spreadsheets, database applications, and healthcare management software and tools preferred
Responsibilities
- Oversees all aspects of Patient Centered Dental Home assignment
- Implements the overall Dental Home Strategy, including supporting member and provider outreach, coaching, and education
- Collaborates with project management team on system updates, revisions, or additions to the Dental Home workflow in Roosevelt
- Maintains and updates the Patient Centered Dental Home sections of the provider manual and member handbook
- Monitors and analyzes reports regarding the Patient Centered Dental Home program
- Ensures that members and their families receive preventive services from a trusted primary care dentist
- Facilitates communication between members, dental providers, Managed Care Organizations and other healthcare professionals
- Develops, recommends, implements, and enforces policies, procedures, guidelines, and standards of operations for the Dental Home Strategy
- Assists in periodic compliance efforts and ensures compliance with healthcare regulations and standards
- Develops ad hoc reporting for Dental Home strategy independently or in collaboration with internal reporting teams
- Directs tracking of provider terminations and reassigns members to another Dental Home as necessary
