Requirements

  • Update documentation for patients
  • HV documentation upload within 48 hours
  • Collaborate and manage a caseload of 50-75 patients
  • Provide necessary training to patients, home health aides, and nurses as required
  • Weekly calls
  • Conduct visits to patient’s homes to build and strengthen relationships with the patient and their family members.
  • Conduct initial set up home visit for patients newly approved for home care and provide information on the program, as well as facilitate caregiver onboarding
  • Attend patient doctor appointments when needed, providing support, and advocating for suitable home care and DME
  • Some payroll assistance for contractors and Case Management
  • Compliance paperwork as needed from contractors
  • Report and document incident reports
  • Assist in the preparation and processing of patient assessments and care plans.
  • Coordinate with providers to secure approval for the patient’s appropriate durable medical equipment (DME)
  • Collaborate with patients and their healthcare providers to oversee medical documentation for their home care renewals every six months.

Responsibilities
