Requirements
- A Bachelor of Science Degree in Nursing
- A compact unrestricted registered nurse (RN) license
- At least 5 years of nursing experience
- 3 years in solid tumor oncology nursing
- Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) required or obtained within 2 years of hire.
Responsibilities
- Conduct comprehensive clinical assessments
- Oversee member health
- Facilitate end-of-life care discussions
- Engage with members and address clinical issues
- Manage escalations that arise
- Familiarize with Thyme Care systems, tools, technology, and partners
- Conduct a minimum of 20 member calls per day
- Collaborate closely with Nurse leaders and Medical Directors
- Establish trusting relationships with members and their care network
- Adhere to Care Team policies, procedures, and documentation standards
- Support members throughout the oncology care continuum
- Identify and address member needs promptly
- Participate in case conferences to monitor member progress
- Foster strong partnerships with payers and providers
- Collaborate with non-clinical Care Team members
- Be available for urgent clinical escalations
- Perform other projects and duties as assigned
Preferred Qualifications
- Compact + single state licensure in New York, Illinois, Michigan, and/or California licensure is preferred.
