Requirements
- Graduate of an accredited school of nursing
- RN-Registered Nurse of California
- BLS-Basic Life Support Healthcare Provider
- 5 years recent relevant experience
Responsibilities
- Offer individualized assistance to patients, families, and caregivers
- Collaborate with physicians and members of interdisciplinary teams to triage, coordinate, and manage patient care
- Educate patients and serve as a point of contact for patients and families
- Participate in strategic initiatives within the clinical program or service line
- Develop, implement, and monitor navigation activities and sustainability
- Facilitate shared decision making and patient engagement
- Address patients’ communication and health literacy needs
- Help patients integrate their values and preferences into treatment decisions
