Requirements
- Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2024 and May 2025.
- Currently residing in or willing to relocate to program location for at least the next two years.
- Have not yet worked as an NP.
- Minimum of one year experience working as an adult RN.
- Minimum GPA of 3.0 in NP program.
- Currently holds or will hold by 9/15/2025 an active, non-probationary state NP license.
- A genuine passion for adult primary care.
- A supportive attitude toward our patient population of older adults.
- Embrace teamwork and the opportunity to collaborate with brilliant colleagues.
- Want to be a part of an innovative model focused on empirically guided population health.
Responsibilities
- Building their own patient panel.
- Learning from a multitude of preceptors.
- Engaging in didactic content that aims to build on, not repeat, graduate training.
- Committing to work at Oak Street for at least a year following program completion.
Preferred Qualifications
- Spanish language skills are strongly preferred, depending on clinic location.
