Requirements

  • Graduate of an accredited LPN/LVN program.
  • Active, unencumbered license to practice as an LPN/LVN in this state.
  • Proficient in EMR systems; PCC experience preferred.
  • Ability to assess resident needs and develop care plans.
  • Strong communication, organizational, and problem-solving skills.
  • Knowledge of nursing and medical practices, laws, and regulations.
  • Ability to supervise, lead, and train other personnel.

Responsibilities

  • Administer and oversee resident assessments, set assessment schedules, and ensure accurate and timely completion.
  • Coordinate the development and implementation of care plans in accordance with regulatory requirements.
  • Initiate Medicare coverage for qualifying residents and issue denial notices as needed.
  • Remain up-to-date on Medicare and Medicaid regulations and documentation requirements.
  • Assist with MDS coding and submission, including RAPs and Triggers.
  • Direct the daily activities of nursing assistants in compliance with facility policies and regulatory guidelines.
  • Maintain and update procedure manuals and job descriptions as necessary.
  • Participate in planning services, programs, and quality assurance initiatives.
  • Admit, transfer, and discharge residents; complete required forms and reports.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Maintain accurate and timely charting for admissions, transfers, and discharges.
  • Transcribe physician orders and ensure implementation.
  • Record incident and accident reports per policy.
  • Document changes in diet orders and notify dietary services.
  • Ensure all entries are signed, dated, and compliant with documentation standards.
  • Prepare and administer medications per physician orders.
  • Maintain secure and accurate records of narcotics and other medications.
  • Dispose of medications per facility protocols.
  • Participate in staff evaluations and make staffing recommendations.
  • Support orientation and supervision of new personnel.
  • Conduct regular team meetings to promote communication and performance improvement.
  • Support and supervise CNAs in delivering direct resident care.
  • Make rounds to evaluate resident condition and care compliance.
  • Communicate with physicians, families, and residents regarding health changes.
  • Ensure residents’ dignity, safety, and comfort are maintained.
  • Support training programs related to infection control, safety, abuse prevention, and other regulatory topics.
  • Mentor staff in best practices and facility procedures.
  • Ensure compliance with all safety protocols and PPE use.
  • Participate in infection control planning and incident reporting.
  • Monitor cleanliness and sanitation in resident care areas.
  • Ensure adequate stock and safe use of nursing supplies and equipment.
  • Report equipment issues or hazardous conditions.

Preferred Qualifications

  • Experience with MDS and care planning strongly preferred.
  • PCC experience preferred.
