Requirements
- Graduate from an accredited licensed vocational nursing school
- LVN-Licensed Vocational Nurse
- BLS-Basic Life Support Healthcare
- DL-Valid Drivers License Class C
- AUTO-Automobile Insurance
- 5 years recent relevant experience
Responsibilities
- Facilitates the Home Care and/or Hospice admission process
- Attends hospital rounds
- Facilitates orders
- Assesses patient needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation
- Enters referral into electronic medical record and completes referral process and admits patient as appropriate
- Forwards hospital documents to hospice and communicates with hospice and hospital staff
- Provides education to hospital staff and physicians
- Makes home visits to provide patient care coverage as the business need requires
