Requirements
- Licensed Practical Nurse (LPN) license issued by the state in which the team member practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
- A valid drivers license issued by the Division of Motor Vehicles
- Advanced training beyond High School that includes the completion of an accredited or approved program in Practical Nursing
- Typically requires 1 year of experience in performing assessments and providing treatments
- Demonstrated knowledge and skills to collect data relative to the patient’s status, and interpret and report the information
- Ability to interact well with other team members, patients, physicians and family members of the patient
- Excellent communication skills
- Strong interpersonal skills and the ability to work with a variety of clients with patience and respect
- Basic computer skills including the ability to document and access information in an electronic system
- Must have a tolerance for differences and an appreciation of multi-culturalism and diversity of the patients and their families.
Responsibilities
- Performs basic assessments, collects data, identifies problems and follows a care plan developed by the Registered Nurse.
- Administers medication and treatments. Provides wound care, hot/cold compresses, patches, G-tube, obtains specimens, takes patients vitals, and performs other medical procedures as documented in the plan of care.
- Communicates with Registered Nurse on the status of the patient after each visit and any changes in their patient’s condition. Communicates any problems and documents the plan and actions taken in the patient’s visit report.
- Follows up with the patient’s physician as needed, including taking verbal orders.
- Participates in chart audits, Continuous Quality Improvement activities and in services as required. Records observations completely, accurately, legibly and concisely.
- Documents all information regarding the patients. Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines.
- Communicates effectively with patient, family, or significant others and provides them with necessary emotional support.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
