Responsibilities

  • Work independently with hospital staff to provide outreach to individuals experiencing homelessness and using the emergency department; outreach will include and is not limited to providing linkage to services, reconnection to services, and enrollment in available programs.
  • Work with hospital staff to develop strategies and provide trainings to increase awareness of the program and use of referral system.
  • Track documentation internally and externally using spreadsheets, HMIS, CHAMP, Vertical Change, and other program reporting systems.
  • Ensure all services and encounters are entered into data systems for each participant served on hospital grounds.
  • Complete program specific assessments and data reports in preparation for meetings with funders.

Preferred Qualifications

  • Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
