Requirements
- A Bachelor’s degree in a Behavioral Science, Social Science, or a closely related field
- A Bachelor’s degree in any field and one year of human services related experience
- An Associate’s degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience
- Three years of human services related experience
- Bilingual Spanish language skills
Responsibilities
- Assess the physical and cognitive needs of Oregonians applying for, or currently receiving, long-term care services
- Determine financial eligibility for some long-term care programs
- Develop, coordinate, and arrange a variety of services supporting older adults and people with disabilities
- Help families in Oregon’s family assistance programs to become self-sufficient while helping them meet their basic needs
- Help develop and implement case management plans
- Coordinate the provision of services for community-based care and nursing home care
- Regularly review and update long-term care service eligibility
- Monitor social service plans
- Identify and mitigate safety risks
Preferred Qualifications
- Experience with managing services for individuals
- Experience interpreting and implementing policy and procedures
- Experience preparing and maintaining written narrative documentation of actions taken
- Experience interviewing to obtain information to evaluate the next steps to take in a process
