Responsibilities
- Determines patient eligibility for home care and hospice care.
- Practices nursing according to Franciscan Alliance and nursing division’s philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. Evaluates one’s own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations.
- Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Collaborates with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.
- Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care. Responds to patient reassessments and responses to care plan.
- Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously reviews and revises the plan of care as patient condition warrants.
- Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment.
- Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient/family utilizing available data/resources.
- Communicates timely changes in patient’s assessment and ongoing reassessment to providers that warrant consideration and/or a change in the plan of care. Maintains and coordinates communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery. Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner.
- Promotes patient independence by establishing patient care goals, teaching patients, friends, and families to understand the condition, medications, and self-care skills. Answers questions to support knowledge and understanding.
- Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols. Administers and stores medication according to controlled substance regulations.
- Ensures operation of equipment by completing preventive maintenance requirements as per manufactures instruction or calling for repairs when required. Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory.
