Requirements

  • Associate’s Degree- Nursing – Required
  • Registered Nurse (RN)- State Licensing Board – Required
  • Basic Life Support Program (BLS) – American Heart Association – Required
  • 1 year Home Health or Hospice Care Experience – Required
  • 2 years Long Term Care Experience- Required

Responsibilities

  • Determines patient eligibility for home care and hospice care.
  • Practices nursing according to Franciscan Alliance and nursing division’s philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Collaborates with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.
  • Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care.
  • Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review and revise the plan of care as patient condition warrants.
  • Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment.
  • Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient/family utilizing available data/resources.
  • Communicates timely changes in patient’s assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.
  • Promotes patient’s independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions.
  • Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance to infection-control policies and protocols. Administers and stores medication according to controlled substance regulations.
  • Ensures operation of equipment by completing preventive maintenance requirements as per manufactures instruction or calling for repairs when required. Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory.
