Requirements
- Must be a Registered Nurse (RN)
- Experience in hospice or palliative care
- Ability to develop and maintain therapeutic relationships with patients and families
- Strong documentation and communication skills
- Flexibility to work on-call during holidays
Responsibilities
- Provide hospice services and establish relationships with patients, families, and caregivers
- Coordinate medical needs of patients
- Assess, plan, implement, and evaluate nursing care for assigned patients
- Manage pain and symptoms for patients
- Document and connect with the care team through point of care documentation
- Make visits to private residences
