Requirements
- Graduate from an approved school of nursing
- Current Ohio License
- Must maintain current Ohio Licensure
- Must maintain CPR certification
- Must maintain CCTC certification
Responsibilities
- Coordinate follow-up care for heart and lung transplant recipients
- Act as a liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies, and other support services
- Provide patient education as indicated
- Assess data reflective of the patient’s status and formulate an appropriate course of action in collaboration with the transplant team
Preferred Qualifications
- BSN preferred
- Prior transplant experience preferred
- Current Certified Clinical Transplant Coordinator (CCTC) certification preferred
