Requirements
- Education: FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
- Licensure: Hold a valid license in the State of Indiana
- Current DEA licensure / Prescriptive Authority post-hire
- Certification: Nurse Practitioner certification from either the ANCC / AANP or Physician Assistant certification from NCCPA
- Certified in Basic Life Support (BLS)
- Experience: 2 + years of clinical experience as an NP/PA in the primary care setting
Responsibilities
- Manage patients of all ages with a variety of health conditions
- 35 patient hours and 5 administrative hours per week
- A supportive work environment with a focus on work-life balance
- Flexible schedule (4-day work week)
- 100% outpatient
Preferred Qualifications
- Spanish speaking skills are preferred but not required
- Previous experience working in a Community Health Center is preferred but not required
