Responsibilities
- Identify, engage, and formalize partnerships with organizations that serve potential referral populations
- Leverage market intelligence and patient population data to strategically target high-potential partners
- Collaborate with internal stakeholders to align outreach strategies with organizational priorities
- Serve as the primary liaison between external referral partners and internal clinical and operational teams
- Ensure seamless handoffs, timely communication, and adherence to partnership agreements and referral workflows
- Monitor performance metrics and partner feedback, proactively resolving issues and identifying opportunities to improve service delivery and partnership outcomes
- Develop and manage relationships with external data sources to support timely identification of eligible patients
