Requirements
- Master’s Degree in Speech/Language Pathology
- Speech-Language Pathology licensure (SP) in the State of Ohio
- Certification of Clinical Competence (CCC-SLP) from the American Speech Language Hearing Association (ASHA)
- Basic Life Support (BLS) certification
- Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter
- Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter
Responsibilities
- Perform initial and on-going assessments of patient’s condition and establishes goal and plan of care which are appropriate to problems identified and involves the patient/family according to their capabilities and desires
- Perform therapy interventions using standard speech therapy techniques and skills appropriate for the patient’s age and condition
- Evaluate communication and swallowing function of patients
- Select assistive speech devices to improve patient’s communication
- Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions
- Participate in ongoing interdisciplinary assessment of assigned patients
- Participate as a member of the hospice interdisciplinary team when necessary
Preferred Qualifications
- One year of experience as a Clinical Speech Language Pathologist
