Requirements
- Registration in good standing as a RPN/LPN with provincial governing bodies
- Minimum two years’ experience as a practicing nurse
- Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices
- Knowledgeable in best practice standards for community health care and services
- Strong oral and written communication skills
- Excellent organizational skills
- Demonstrated competency in patient centered care principles and practices
- Knowledgeable of community resources available for patients and families
- Strong independent critical thinking and problem-solving skills
- Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
- Access to a personal vehicle required
Responsibilities
- Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services
- Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions
- Perform the initial personal care services for new patients
- Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements
- Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care
- Maintain current and accurate patient records
- Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff
- Stay abreast of changes in practice setting that affect scope of practice
