Requirements
- Bachelors degree in Social Work, Counseling or a related field required
- At least 2 years of experience in assessments, social work or related field
- Any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job.
Responsibilities
- Provides consultation and information to clients, volunteers, service providers and the general public regarding programs and activities related to senior services
- Assesses, screens and determines client referrals for service eligibility; conducts interviews, needs assessments and home visits
- Develops, implements and monitors comprehensive plan of care for clients based on assessment, client expectations, and availability of services
- Assists general public and clients with locating and applying for financial and material assistance, completing applications; information and submitting documentation to agencies
- Maintains client case files in departmental database; documents client visits, services provided, client progress
- Participates in special projects or programs of the assigned center, delivers meals and care packages to homebound clients and takes applications for assistance programs
- Maintains a comprehensive, current knowledge of applicable policies, procedures, regulations, codes, and other regulatory standards related to aging services
Preferred Qualifications
- Experience entering information into the DeKalb Homeless Management Information System- ECCOVIA Solutions’ ClientTrack system
- Experience in working/collaborating with homeless agencies that provide HUD funded homeless services
