Requirements
- Masters degree in Social Services
- Certified Social Work (CSW) – South Dakota Department of Social Services
Responsibilities
- Coordinates and evaluates care options for patients
- Assesses the needs of patients and partners with the multidisciplinary team and patient/family to develop a care plan
- Stays current with federal/state regulations to ensure compliance
- Supports patients and families in understanding and following medical recommendations
- Identifies psychosocial, historical, financial, cultural, and spiritual needs
- Evaluates current treatment plans and identifies barriers
- Partners with providers and Physician Advisors/Medical Directors on complex cases
- Completes evaluations of hospitalized patients and customizes their plans
- Maintains knowledge of community resources and payer benefits
- Adheres to federal/state and regulatory standards
- Assists patients in maintaining competency and identity
Preferred Qualifications
- 3+ years of Healthcare Experience
