Bilingual Analyst – Case Management – Field – Sign on Bonus

Requirements

• Candidate must reside in Miami Dade County, FL (Applicable Zip codes: 33125, 33130, 33133)
• Must possess reliable transportation and be willing and able to travel up to 75% of the time to meet members face to face in Miami Dade county and surrounding areas in FL. Mileage is reimbursed per our company expense reimbursement policy
• Bi-lingual, English, and Spanish
• 1 + year of experience in behavioral health or a long-term care setting
• Computer proficiency in Microsoft Word, Excel, and Outlook

Responsibilities

• Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness
• Effectively manage a caseload that includes supportive and medically complex members
• Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration
• Determine appropriate services and supports due to member’s health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Preferred Qualifications

• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed care experience
• Case management and discharge planning experience
