Requirements
- Master’s degree in social work from a CSWE accredited program
- State Licensed Social Worker (LSW)
- Two years of experience in a hospital or healthcare setting
- Licensed Independent Social Worker (LISW) within five years of hire
Responsibilities
- Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems
- Perform psychosocial assessments and provide interventions to assist patients and families, including counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings
- Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans
- Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers
- Maintain a network of resources for consultation and connect patients with community resources
- Supervise social work interns and assist with training new caregivers
- Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes
