Requirements

  • Nursing degree (LPN or RN)
  • Minimum 3-4 years of experience in health care setting
  • Minimum one year of experience in quality management, preferably in medical clinic practice setting

Responsibilities

  • Works in collaboration with the Division Chief of Primary Care to meet the documentation goals of Population Health and the Phelps Selected ACO.
  • Is embedded in the ambulatory clinics weekly to support the efforts of Population Health and the ACO.
  • Responsible for supporting and working with patient communities and is the contact for all ACO related content and population health data connected to the ACO.
  • Monitors medical practice efforts to ensure compliance with the documentation goals of the ACO.
  • Shares responsibility for quality work and initiatives. With the Division Chief of Primary Care is the contact for all Quality related content across the ACO.
  • Responsible for working Care Gap Closures and with Clinic Manager assistance builds a process for closures.
  • Responsible for working medication adherence opportunities across the ACO and payor contracts.
  • Coordinates the effort to gather data and prepare reports to meet the goals of the ACO.
  • Monitors service quality through ICD-10 review, HEDIS quality measures, ACO quality initiatives, payer/provider/patient satisfaction surveys, and complaints. Follows through on complaints at the clinical level. Reports on results.
