Responsibilities
- Performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program.
- Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
- Effectively collaborates with other healthcare providers in coordinating and implementing the patients plan of care, to include evaluation, treatment, and surveillance.
- Effectively communicates with patient via mail, telephone, and in person regarding information pertinent to their care.
- Assures pertinent patient information is received and available for provider interaction with patient.
- Coordinates patient needs for the program.
- Serves as the point of contact for providers.
- Coordinates patient activities, such as support groups, educational series, as appropriate.
- Provides limited patient and family education based on individual needs either in person or by telephone.
- Coordinates patient activities through available support services, as appropriate.
- Maintains systems and processes sufficient to assure timely follow-up and surveillance of patients for both Programs.
- Serves as a liaison with Scheduling Services to ensure appropriate scheduling of patients.
- Develops and implements patient screening tools and efficient clinic procedures.
- Assists in the development and implementation of efficient patient navigational procedures.
- Investigates patient concerns including appointment access, communication, and billing.
- Maintains clinical supply inventory.
- Responsible for conducting medical record reviews to report findings to physician specialist.
- Provides nursing care and assists physician and procedures.
- Routinely inspects and observes condition and operation of equipment to ensure proper operation and identify the need for preventive and corrective maintenance per guideline.
- Performs ongoing maintenance of exam and treatment rooms.
