Requirements
- Master’s Degree Social Work
- Within 18 months of hire: Certified Case Manager
- Upon hire: Specific Training in Mental Health First Aid/Suicide Prevention or equivalent – current and maintained ongoing
- 5 years Clinical experience
- 2 years In a Behavioral Health setting
- Utilization review/case management
Responsibilities
- Coordinates care for Pacific Medical Centers’ Managed Care patients to assure cost-effective, appropriate utilization of health care resources
- Manages, directs, and coordinates the entire spectrum of care for patients throughout a variety of settings, both inpatient and outpatient
- Assists physicians (internal and external) with management and coordination of care
- Works at varied clinic and provider locations as needed
