Requirements
- Bachelor’s degree
- RN or other professional healthcare licensure
- Five years recent clinical experience applicable to the population preferred
- Knowledge of clinical practice guidelines
- Knowledge of computer databases
- Knowledge of payer protocols
Responsibilities
- Facilitates patients through the health care system from pre-admission through post-discharge coordination of care
- Monitors and coordinates the patient plan of care
- Enhances the timeliness of patient care decisions and throughput
- Enhances care quality and efficiency and ensures efficient resource utilization
- Identifies and removes barriers to progression of the plan of care for each patient
- Collaborates with physicians, nurses, and other health care workers to manage patient transition to the next level of care
- Acts as the central information source for third party payers, communicating the plan of care and medical necessity
- Assists physicians in ensuring the appropriate level of care for each patient
Preferred Qualifications
- Master’s degree preferred
