Requirements
- Master’s Degree in Social Work, Counseling or Marriage and Family Therapy required
- LSW, PC MFT Required. Current license with the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board
- CPI required by end of probationary period
- CPR required by end of probationary period
- A minimum of three years post-Masters degree of experience in a health care setting, mental health setting and/or Alcohol/Drug Treatment Program agency setting is preferred
- Strong Communication and Clinical Skills, including competency in diagnosing, along with knowledge of community resources
Responsibilities
- Provides direct clinical intervention, transition planning and counseling to patients and their families
- Assesses the patient’s social, emotional, financial, behavioral, cultural, and family situation
- Interprets assessment information to the physician, nursing staff, and other care team members to facilitate appropriate transition planning
- Assists patients and families in making appropriate linkages with community resources
- Provides consultation to ER staff and the community regarding patient issues
Preferred Qualifications
