Requirements
- Clinical Licensure or Education Master’s degree required and one of the following unrestricted TN licensures: LCSW or LMSW
- Valid Driver’s License required
- 3 years – Experience providing care coordination to persons receiving long-term care and/or home and community based services required
- Must be detailed oriented, deal well with ambiguity, demonstrate strong strategic thinking and creativity skills, be an effective communicator with both internal and external partners/customers
- Awareness of the impact of cultural/socioeconomic status pertaining to the use of healthcare and mental health care services
Responsibilities
- Providing crisis intervention and helping families to understand the complexities and impact of the medical situation
- Assessing patient and family’s needs for, and responses to, healthcare services on an ongoing basis
- Interviewing patients and planning activities to meet the social and emotional needs of them and their families, and to facilitate the recovery psychologically, emotionally and physically
- Coordinating with external agencies and home care nursing group for post-discharge care
- Must be able to perform home visits if necessary
