Requirements
- Bachelor’s Level Degree or equivalent combination of education and experience with a Major in nursing
- Graduate from an approved school of nursing
- Current Ohio License
Responsibilities
- Coordinate the entire transplant process from day of referral to time of transplant for potential heart and lung transplant candidates
- Organize, collate and guide the medical/surgical data and pre-operative studies in collaboration with the transplant team
- Serve as liaison for transplant team with pulmonologists, cardiologists, consulting services, dietetics, social services and insurance company representatives/case managers
- Develop and conduct educational programs for patients and medical & nursing staff
- Coordinate and submit statistics to national organizations and government agencies
- Assist with development and implementation of new processes/procedures within the program
- Participate in Patient Selection Committee and CTC staff meetings on a regular basis
- Demonstrate the knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served
- Assess data reflective of the patient’s status and formulate an appropriate course of action in collaboration with transplant team
Preferred Qualifications
- BSN preferred
- Prior transplant experience preferred
- Current Certified Clinical Transplant Coordinator (CCTC) certification preferred
- Flexible, sensitive and adaptable to transplant activities
