Requirements
- Minimum of Associate’s Degree in Nursing
- Must meet Qualified Mental Health Professional status in relevant programs
- Active unencumbered Illinois Registered Nurse required
- Effectively communicate via written, verbal, in person and virtual methods
- Skilled in customer service techniques
- Skilled in problem-solving
Responsibilities
- Provides life skills and wellness didactic services utilizing evidence-based methodologies to provide excellent clinical services
- Prepare for and conduct individual, group, and family therapy for assigned caseload
- Assist service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered
- Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned
- Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials
- Assist service recipients with making linkage with internal and external resources as outlined in treatment plan
- Assist service recipient in making discharge plans and provide follow up medical services as assigned
- Administers and gives medications to patients, monitors the dosage, side effects, use and documents patients’ reactions
- Provides medication training to the client or client’s family or guardian to administer the client’s medication
- Obtains laboratory specimens and help doctors with patient examination and continuity of care arrangements with other providers
- Monitor urinalysis, breathalyzer, and other testing as assigned
