Requirements

  • Graduate from an accredited school of nursing (RN or LPN)
  • Current LPN or RN license in the State of Missouri or Compact State
  • Certified MDS Assessment Coordinator preferred or completion of within 6 months of hire
  • American Heart Association Basic Life Support

Responsibilities

  • Follows established hospital policy regarding patient safety measures such as infection prevention, safe patient handling and movement, and notifies nursing staff accordingly if safety issues noted to insure that patient care is provided in an optimally safe environment.
  • Provides quality MDS assessments to collect all information required and establishes a positive working relationship with patient and family.
  • Performs ongoing MDS assessments per guidelines and insures appropriate signatures are obtained from all required disciplines.
  • Documents accurately and efficiently all patient care activities and required quality monitoring items to support the achievement of compliance with established quality measures.
  • Communicates to physicians and appropriate team members pertinent patient information in order to improve patient outcomes and quality care.
  • Demonstrates understanding of the dynamics of health care, adapting to change in a positive and professional manner in an attempt to support the hospital mission, vision, and values.

Preferred Qualifications

  • Two years of experience with geriatric population preferred
  • Certified MDS Assessment Coordinator preferred or completion of within 6 months of hire
