Requirements
- Bachelor’s Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
- Experience gained through completion of a pharmacy degree program
- LIC-Clinical Pharmacist – STATE_MI State of Michigan 120 Days required
- LIC-Pharmacist Controlled Substance – STATE_MI State of Michigan 120 Days required
- CRT-Basic Life Support (BLS) – AHA American Heart Association 180 Days required
- CRT-Basic Life Support (BLS) – ARC American Red Cross 180 Days required
- CRT-Pediatric Adv Life Support (PALS) – AHA American Heart Association 180 Days required
- CRT-Adv Cardiovascular Life Support (ACLS) – AHA American Heart Association 180 Days required
Responsibilities
- Provide safe and effective patient care services
- Promote safe and effective access to medication therapy
- Provide patient-centered care through interacting and facilitating effective communication with patients, caregivers, and stakeholders
- Participate in the identification and implementation of medication-related interventions for a patient population (population health management)
- Conduct practice advancement projects
- Demonstrate leadership skills that contribute to departmental and/or organizational excellence in the advancement of pharmacy services
- Demonstrate leadership skills that foster personal growth and professional engagement
- Provide effective medication and practice-related education
- Provide professional and practice-related training to meet learners’ educational needs
- Complete all additional activities required and outlined by individual Pharmacy Residency Program
