Requirements
- Bachelor’s Degree Science of Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
- 2 years of relevant pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Required
- LIC-Clinical Pharmacist – STATE_MI State of Michigan 90 Days required
- LIC-Pharmacist Controlled Substance – STATE_MI State of Michigan 90 Days required
- CRT-Basic Life Support (BLS) – AHA American Heart Association Ambulatory Sites 90 Days required
Responsibilities
- Provide chronic disease state management and/or medication therapy management directly to patients within the Clinic setting
- Build relationships with providers and staff by living our values to promote trust and gain referrals within the clinic
- Follow Spectrum Health’s standard work, policies and procedures to gather and interpret patient vitals and labs during visits
- Participate in team-based approaches to help determine the best treatment plan for the patient
- Collaborate with providers and office staff on a regular basis to facilitate educational updates on guidelines, new medications or therapies, and best practices
- Assist clinic in monitoring and improving upon system Quality Measures including, but not limited to, MTM, A1c, Hypertension, Adherence, and Readmissions
- Help patients and clinic staff navigate formularies and other programs to assist in lowering the cost of care for patients and the system
- Engage in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities
- Precept pharmacy residents and students independently as assigned
- Evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels, using pharmacokinetic principles
- Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests
- Coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned
- Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency
- Facilitates applicable Pharmacy Committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair as assigned
- Coordinates the development, editing, and dissemination of departmental publications for physicians and hospital staff
Preferred Qualifications
- 2 years of relevant experience hospital pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Preferred
- ASHP accredited postgraduate year 2 (PGY2) residency in area of specialty Preferred
- CRT-Adv Cardiovascular Life Support (ACLS) – AHA American Heart Association 180 Days preferred
- CRT-Board Cert Pharmacotherapy Specialist (BC-PS) – ASHP American Society of Health-Systems Pharmacists Upon Hire preferred
- CRT-Board Cert Pharmacotherapy Specialist (BC-PS) – BPS Board of Pharmacy Specialties Upon Hire preferred
- Ideal candidate will be a Pharmacist with either a PGY1 Residency or PGY2 Residency in Family Medicine, Internal Medicine, or Community Pharmacy, or a Pharmacist possessing 3+ years of experience in Retail/Community Pharmacy (in lieu of Residency)
