Requirements
- Completion of an approved program per state’s requirements.
- Related Field – 1 year of experience
- Licensed Practical Nurse – Arkansas State Board of Nursing (ASBN)
Responsibilities
- Complete referral to community resource agencies for patients with home discharge needs.
- Coordinates and communicates the arrangement of the discharge planning activities.
- Documents agencies who will be providing services in the medical record along with the completion of follow-up phone calls.
- Completes discharge follow-up phone calls and documents in medical record. Communicates any relevant gap in providing care to a patient that is identified during the phone call.
- Assist the manager in writing appeal letters for denials of in-patient admissions.
