Requirements

• Nursing degree from an accredited school; bachelor’s preferred.
• Minimum of 1 year of hospice care experience, plus 1+ years in acute and home care settings.
• Active state nursing license and BLS CPR certification.
• Strong assessment, communication, problem-solving, and nursing skills.
• Ability to lift, transfer, and position patients; frequent movement required.
• Valid driver’s license with insured, reliable transportation.

Responsibilities

• Assess patients’ physical, psychological, social, cultural, and spiritual needs.
• Develop individualized care plans focused on symptom management and quality of life.
• Coordinate care with interdisciplinary team members for comprehensive support.
• Educate patients and families on hospice philosophy and end-of-life care.
• Monitor patient progress, adjusting care plans as needed.
• Facilitate seamless communication to ensure continuity of care.
• Conduct follow-ups to maintain care compliance and resolve concerns.
• Identify and address barriers to successful care transitions.
• Advocate for patient needs from pre-admission through discharge.
• Collaborate with clinical teams to source hospice referrals.
• Review Explanation of Benefits (EOBs) and consent documents with patients.
• Perform nursing assessments to evaluate hospice eligibility.
• Admit qualifying patients for general inpatient hospice care.
• Support hospice service line through sales, marketing, and public relations efforts.

Preferred Qualifications

• Preferred experience in holistic or alternative care modalities.
